Developing a Patient-Centred Measure of Self-Management for Individuals with Multiple Chronic Conditions (PRISM-CC)

Advances in chronic disease management are dependent on the ability to move beyond “one size fits all” interventions to specify clients’ ongoing and changing needs over the trajectory of their disease(s). Yet, we still don’t have the assessment tools needed to help patients and their providers identify self-management skills and priorities, or to evaluate self-management outcomes. Our goal is to develop a measure of self-management that will become the primary instrument for both clinical care and research with patients with multi-morbidity. The version 1 of the Patient Reported Inventory of Self-management of Chronic Conditions (PRISM-CC) will have sufficient clinical specificity to tailor care and measure patient outcomes.

The development of PRISM-CC follows the PROMIS Instrument Development and Validation Scientific Standards; however, calibrated items will not become part of the PROMIS item bank nor subject to licensing. PRISM-CC is based on a conceptual framework developed by our team. Data is collected and analyzed using confirmatory factor analysis to validate the domains. Items are calibrated and evaluated using Item Response Theory.

The PRISM-CC will improve our ability to assess which patient and provider supports are essential to improve self-management for individuals with multiple chronic conditions. It will help clinicians to tailor care for each individual and researchers to improve the effectiveness of self-management interventions and services.
